What constitutes a"failed" arch bridge

Some discussion on the Civil Engineering Heritage Exchange over ther past couple of days about monitoring, measurement and testing. One comment questioned what I meant by arch bridges having "failed" under loads noticeably less than the assessed capacity.

I think this bridge has been loaded beyond its servicability limit!
